Chronic diseases are the major driver in healthcare costs in the US and globally.  Diseases like Diabetes, Hypertension and Asthma account for the majority of deaths and costs in the US.  Managing these diseases can be a challenge, as much of the burden falls upon the patient to self-manage them on a daily basis. Specifically, managing the dosing and timing of multiple medications is a challenge that results in both poor adherence and outcomes in many of these patients.

More than 23 million people in the US have been diagnosed with diabetes, costing the health system $327 billion per year. Approximately 1.8 million people have Type 1 diabetes, an autoimmune condition where the body attacks and destroys its own insulin-producing cells, requiring the patients to self-monitor their blood glucose and self-administer insulin multiple times a day for the rest of their lives. Over 21 million people have Type 2 diabetes, a condition where the patient's insulin becomes less effective, often through normal aging and/or increased body weight.

It is a progressive disease, for which approximately 1/3 of Type 2 diabetes patients must self-monitor their blood glucose, and self-administer insulin on a daily basis in order to maintain blood glucose control.

Studies have shown that patients who maintain relatively tight blood glucose control, as indicated by a lab test measuring HbA1C (glycosylated hemoglobin), can drastically reduce the risk of devastating diabetes-related complications such as nephropathy, retinopathy and neuropathy. Nephropathy can result in dialysis, retinopathy can result in blindness and neuropathy can result in foot ulcers and amputations.

Poor, or non-adherence to medication for chronic diseases in the US alone account for nearly $300 billion in costs to the healthcare system. Countless physician visits, hospitalizations and ER visits could be avoided if patients were able to take their medications appropriately.

This is especially true in diabetes, the first focus area for Chronicare. For example, more than 7 million people with Type 1 and Type 2 diabetes inject insulin on at least a daily basis. Several million of them take multiple injections each day, but often struggle with their self-management for reasons such as:

Self-Management Challenges

As a result, an estimated regimen, resulting in very poor outcomes and a substantial financial burden on the healthcare system. 93% of health plans report having implemented a medication adherence program for diabetes, yet patients are still unable to adhere to their medication regimens.

Type 1 Patients
Chronicare can provide Type 1 diabetes patients on multiple daily injections with many of the benefits experienced by patients on insulin pump therapy, but at a fraction of the cost. Our data-driven tools and certified diabetes educator health coaches can assist with insulin dosing ratios for carbohydrate intake and blood glucose corrections, basal rate adjustments, bolus calculation and active insulin.
Type 2 Patients
For patients initiating basal insulin, Chronicare can help guide the patient along their insulin titration journey. Using glucose and insulin data, the system will provide suggested dosing changes powered by clinically validated dosing algorithms. For patients on multiple daily injections, our data-driven tools and certified diabetes educator coaches can assist with insulin dosing ratios for carbohydrate intake and blood glucose corrections, basal rate adjustments, as well as overall medication management.
